This is important because it separates two very different ideas. One idea is biochemical target engagement, meaning a compound changes a measurable marker in the body. The other is a meaningful clinical outcome, meaning people actually experience a clear, reliable benefit in function, healthspan, cognition, metabolism, or daily wellbeing.

Right now, the evidence is stronger for the first than the second. NAD+ support can influence markers. The harder question is when that translates into meaningful results for specific people in specific situations.

NR and NMN can raise NAD+ markers, but results are not uniform

Nicotinamide riboside, often called NR, and nicotinamide mononucleotide, often called NMN, are two of the best known NAD+ precursors. These compounds are studied because the body can use them through NAD+ related pathways.

A July 2026 phase II trial looked at NR in older adults with amnestic mild cognitive impairment. The study found that NR doubled blood NAD+ levels and was well tolerated over 12 weeks, with no serious adverse effects reported. However, it did not improve the primary cognitive outcome, total cerebral blood flow, or blood pressure over that period. Exploratory analysis suggested possible regional blood flow changes in the hippocampus, but that needs longer and larger studies.

A July 2026 systematic review and meta analysis focused on oral NMN in adults. It found that short term oral NMN or NMN related supplementation showed favorable tolerability, with no clear increase in adverse events or liver enzyme abnormalities. At the same time, broad metabolic benefits were not clearly demonstrated across common markers like body weight, glucose, lipids, and systolic blood pressure. Some signals, such as diastolic blood pressure and HOMA IR trends, were described as exploratory and in need of confirmation.

  • NR Nicotinamide riboside — a form of vitamin B3 studied as an NAD+ precursor. Human research suggests it can increase blood NAD+ or NAD related markers, although changes in clinical outcomes are not always consistent. Recent studies continue to explore NR in areas such as brain health, metabolism, cardiovascular function, and cellular energy.
  • NMN Nicotinamide mononucleotide — another NAD+ precursor studied in human trials and meta analyses. Recent research suggests short term oral NMN is generally well tolerated in adult randomized trials, while broad metabolic benefits remain uncertain and need larger, longer studies. It is one of the most discussed compounds in the NAD+ and healthy aging category.

The biggest shift is toward precision

One of the most interesting developments is that NAD+ research is moving away from simple “take this and feel younger” thinking.

Recent work suggests that NAD+ responses may depend on dose, route of administration, age, metabolic state, baseline health, tissue type, gut microbiome activity, and individual biology. That means the same NAD+ precursor may not behave the same way for every person.

Another important 2026 finding is that whole blood NAD+ may not always tell the full story. A Nature Metabolism study reported that whole blood NAD+ levels stayed relatively stable with age and lifestyle interventions, while still responding to nicotinamide riboside supplementation. This challenges the simple marketing idea that blood NAD+ automatically declines in everyone and that a single blood marker explains healthy aging.

Research is also becoming more interested in where NAD+ is changing. Blood, muscle, brain, immune cells, and mitochondria may not all tell the same story. Future trials will likely need better tissue specific and function specific endpoints.
