Inflammation: The Quietest Lever in Longevity, and How to Pull It
Most of us picture inflammation as something dramatic — a swollen ankle, a red throat, a pulled muscle. But the inflammation that quietly shapes how you age isn't loud at all. It's a low hum sitting in the background of your blood chemistry for years, and the link between inflammation and longevity turns out to be one of the most consistent predictors of how the rest of your life unfolds.

This is where the relationship between inflammation and longevity becomes genuinely useful rather than abstract — because unlike most of the markers longevity science obsesses over, this one is cheap to measure and surprisingly responsive to the ordinary things you already control. None of what follows is medical advice, but it may be one of the more worthwhile conversations to have with yourself, and your GP.
What "inflammaging" actually is
The term inflammaging was coined around the year 2000 to describe something biologists kept seeing in older adults: a chronic, low-grade, systemic inflammation that simmers in the background even when there's no infection or injury to explain it. Unlike the helpful inflammation that flares to heal a cut or fight a virus — does its job, then resolves — inflammaging is the kind that never quite switches off.
It shows up in your bloodwork as quietly elevated levels of inflammatory markers such as high-sensitivity C-reactive protein (hsCRP), interleukin-6 (IL-6) and tumour necrosis factor-alpha (TNF-α) — proteins your body produces when inflammation is active. None of these will make you feel unwell day to day. But over decades, this background hum is now understood to drive, or at least travel alongside, many of the biggest age-related conditions: cardiovascular disease, type 2 diabetes, Alzheimer's and other neurodegenerative diseases, certain cancers, and the loss of physical resilience researchers call frailty.
A major review in Nature Reviews Cardiology put it plainly: inflammaging is now treated as a risk factor for cardiovascular disease, and the evidence increasingly points to the association being causal rather than a marker simply tagging along. In other words, inflammation may not be a side effect of ageing so much as one of its engines.
Why inflammation matters more than almost any other longevity marker
Here's the thing about longevity research: most of the famous markers — telomere length, epigenetic "age clocks", mitochondrial function — are either expensive to measure, hard to interpret, or stubborn to change. Inflammation is different on all three counts.
It's cheap to measure: a standard hsCRP blood test costs only a few pounds or dollars and is widely available. It's relatively easy to interpret: lower is generally better, and researchers increasingly suggest that under 1.0 mg/L — ideally closer to 0.5 mg/L — may be a longevity-favourable range, well below the standard "under 3.0 mg/L" cardiology cut-off. And it's strikingly responsive: research suggests meaningful changes to diet, exercise, sleep and body composition can shift hsCRP by roughly 25–40% within months.
Compare that to almost any other longevity lever. Moving your hsCRP by a third in three months isn't optimisation theatre — it's a real change in the direction you're travelling.
What actually drives chronic inflammation
Inflammaging isn't caused by one thing. It's caused by an ecosystem of small drivers that compound over years.
Visceral fat — the deep abdominal fat wrapped around your organs — is metabolically active and releases inflammatory signalling molecules (cytokines) on its own. This is why central weight around the middle is one of the strongest predictors of elevated hsCRP, often more so than total body weight.
Poor sleep is another big one. Even a couple of nights of disrupted sleep can measurably raise inflammatory markers. And if hsCRP stays stubbornly high despite good diet and exercise, undiagnosed sleep apnoea is a common hidden cause — a large share of moderate-to-severe cases (often estimated at around 80%) go undiagnosed.
Diet pattern matters enormously. Highly processed foods, refined seed oils and high sugar intake all tend to push inflammation upward. Mediterranean-style eating — rich in olive oil, oily fish, vegetables, legumes and whole grains — has been shown across multiple trials to reduce hsCRP by around 25–40% over about 12 weeks. That's a similar ballpark to the effect of a moderate-dose medication, achieved through food.
Then there's the long tail: sedentary behaviour, chronic stress, gum (periodontal) disease, gut dysbiosis (an imbalanced gut microbiome), air pollution, and the slow build-up of senescent cells — the so-called "zombie cells" that resist normal turnover. None of these is dramatic alone. The point is that they stack.
How to measure inflammation, and how to move it
Start with a single blood test: high-sensitivity C-reactive protein (hsCRP). It's the most validated single marker of chronic low-grade inflammation, it's inexpensive, and most GPs can order it — or you can use one of the home-testing services that have become widely available. One thing to know about hsCRP: it bounces around. A single high reading after a cold or a hard workout isn't necessarily meaningful. The signal lives in the trend across several measurements, ideally spaced a few months apart.
Once you have a baseline, the highest-leverage moves are reassuringly mundane:
- Eat toward a Mediterranean pattern. More olive oil, oily fish, vegetables, legumes and whole grains; less ultra-processed food, refined oils and added sugar.
- Move daily. Regular exercise has an anti-inflammatory effect through both fat reduction and the direct release of anti-inflammatory myokines — signalling molecules from contracting muscle.
- Protect your sleep. Aim for seven to nine hours, and get screened for sleep apnoea if you snore heavily or wake unrefreshed.
- Lose visceral fat if you're carrying it — this is often where the biggest hsCRP shifts come from.
- Consider the evidence-backed supplements — omega-3s first, then curcumin — as a support to the lifestyle work, not a replacement for it.
hsCRP and related markers are tools for a conversation with a qualified clinician, not a self-diagnosis kit. A persistently elevated result — or any new symptom — deserves proper medical assessment rather than a supplement. If you're pregnant, breastfeeding, taking medication (including blood thinners) or managing a health condition, check with your practitioner before adding omega-3s, curcumin or making major dietary changes.
On the supplement piece specifically: omega-3s (EPA and DHA) have the strongest evidence base, with multiple meta-analyses showing meaningful reductions in hsCRP, IL-6 and TNF-α — the clearest effects landing in the range of roughly 1–2 grams per day of combined EPA + DHA. Curcumin (from turmeric, ideally formulated with piperine or as a phytosome to improve its notoriously poor absorption) has the next-best evidence; it works largely by inhibiting NF-κB, a kind of master switch for inflammatory gene expression. The compounding effect: why it's a lever, not a single fix
Here's why inflammation is worth thinking about as a longevity lever rather than a single disease risk: lowering it tends to move several risk pathways at once. The same Mediterranean diet that drops your hsCRP also tends to improve your blood pressure, your lipid panel, your blood-sugar control and your mood. The same exercise habit that reduces inflammation builds muscle, improves insulin sensitivity and protects bone density.
Inflammation sits so far upstream of age-related processes that pulling this one lever pulls dozens of smaller ones with it. That's the quiet leverage. You don't need a stack of twelve supplements and a longevity-clinic membership. You need to know your hsCRP number, take it seriously, and stack small, consistent inputs over the years that follow.
Standard cardiology guidance treats hsCRP under 3.0 mg/L as "normal." Longevity researchers increasingly aim lower — under 1.0 mg/L, with under 0.5 mg/L as an optimal target. Most people have never been told this number, let alone tested for it. It may be one of the highest-value blood tests almost nobody thinks to ask their GP to run.
- Omega-3 EPA & DHA Of all the supplements with meaningful anti-inflammatory evidence in humans, the marine omega-3s EPA and DHA have the strongest and most consistent track record. Across multiple meta-analyses they're associated with lower circulating hsCRP, IL-6 and TNF-α — largely because they serve as raw material for resolvins and protectins, signalling molecules that help the body resolve inflammation rather than just suppress it. Research suggests the clearest effects sit around 1–2 g per day of combined EPA + DHA, taken with meals over two to three months.
- High-Sensitivity C-Reactive Protein (hsCRP) Not a supplement — it's the protein your liver produces in response to inflammation, and the blood test for it is the single most validated measure of chronic low-grade inflammation in routine use. Standard cardiology guidance calls anything under 3.0 mg/L "low risk," but longevity research increasingly suggests aiming for under 1.0 mg/L, with 0.5 mg/L as an optimal target. The test is inexpensive, widely available, and responds reliably to diet, exercise, sleep and body-composition changes within two to three months.
- Curcumin (from Turmeric) The active polyphenol in turmeric, and after omega-3s it has one of the better evidence bases for anti-inflammatory effects in humans. It works largely by inhibiting NF-κB, the transcription factor that acts as a master switch for inflammatory gene expression. Because plain turmeric absorbs poorly, research-grade studies almost always use it paired with piperine (from black pepper) or as a phytosome preparation. Reductions in inflammatory markers show up most clearly in people with elevated baseline inflammation — and less so in those whose markers are already low.

It's the quiet, daily work of keeping your inflammation low — through what you eat, how you sleep, how you move, and how you manage stress.
Test once to know where you stand. Pick two or three changes you'll actually sustain. Re-test in three months. That's very nearly the whole game — and almost no one plays it. The people who do tend to age rather differently from the people who don't.
